Application Option #2 - Download the PDF version:​

1. Click on the PDF icon below and download the application.

​

2. Fill out an application for each adult in your household on a computer or a printed out copy.

​

3. Collect your recent paystubs or proof of income. You may scan them, forward an email, or take a high quality photos. Job offer letters, SSI, and college/education program letters are often submitted as well.

4. Email the completed applications, along with copies of recent paystubs/proof of income to:
